About the Authors:
Charles F. Lacy, PharmD, FCSHP, is the Vice
President for Information Technologies, Professor of Pharmacy Practice, and
Professor of Business Leadership at the University of Southern Nevada. In his
capacity at the university, Dr. Lacy oversees the directors of the Library and
Leaming Resources Centre and the University Information Systems. Additionally,
Dr Lacy is a co-founder of the University. Currently, Dr Lacy is also the
Director of International Programs for the university, which includes the
College of Pharmacy, the College of Business Administration, and the College of
Nursing. Previously, Dr Lacy was the Facilitative Officer for Clinical Programs
for the College of Pharmacy where he managed the clinical curriculum, clinical
faculty activities, student experiential programs, pharmacy residency programs,
and the college's continuing education programs.
Prior to
co-founding the Nevada College of Pharmacy, Dr Lacy was the Clinical Coordinator
for the Department of Pharmacy at Cedars-Sinai Medical Centre. With 20 years of
clinical experience, he has developed a reputation as an acknowledged expert in
drug information, pharmacy practice, and critical care drug therapy.
Dr Lacy
received his doctorate from the University of Southern California School of
Pharmacy. Presently, Dr Lacy holds teaching affiliations with the University of
Southern Nevada, the University of Southern California, the University of the
Pacific School of Pharmacy, Western University of Health Sciences School of
Pharmacy, Hokkaido College of Pharmacy in Otaru, Japan, and the University of
Alberta at Edmonton, School of Pharmacy and Health Sciences.
Dr Lacy is
an active member of numerous professional associations including the American Society
of Health-System Pharmacists (ASHP), the American College of Clinical Pharmacy
(ACCP), the American Society of Consultant Pharmacists (ASCP), the American
Association of Colleges of Pharmacy (AACP), American Pharmacists Association
(APhA), and the California Society of Hospital Pharma- cists (CSHP), through
which he has chaired many committees and subcommittees.
Lora Armstrong, PharmD, BCPS, He received
her bachelor's degree in pharmacy from Ferris State University and her Doctor
of Pharmacy degree from Midwestern University. Dr Armstrong is a
Board-Certified Pharmacotherapy Specialist (BCPS). In her
current position, Dr Armstrong serves as Vice President of Clinical Affairs
with responsibility for the National Pharmacy & Therapeutics Committee
process, Clinical Program Oversight process, and Pharmaceutical Pipeline
Services at Caremark. Caremark is a prescription benefit management company
(PBM). Dr Armstrong is also responsible for monitoring drug surveillance and
communicating Pharmacy & Therapeutics Committee Formulary information to
Caremark's internal and external customers.
Prior to
joining Caremark, Inc, Dr Armstrong served as the Director of Drug Information
Services at the University of Chicago Hospitals. She obtained 17 years of
experience in a variety of clinical settings including critical care,
hematology. oncology, infectious diseases, and clinical pharmacokinetics. Dr
Armstrong played an active role in the education and training of medical,
pharmacy, and nursing staff. She coordinated the Drug Information Center, the
medical center's Adverse Drug Reaction Monitoring Program, and the continuing
Education Program for pharma- cists. She also maintained the hospital's strict
formulary program and was the editor of the University of Chicago Hospitals'
Formulary of Accepted Drugs and the drug information center's monthly
newsletter Topics in Drug Therapy.
Dr
Armstrong is an active member of the Academy of Managed Care Pharmacy (AMCP),
the American Society of Health-Systems Pharmacists (ASHP), the American
Pharmaceutical Association (APhA), the American College of Clinical Pharmacy
(ACCP), and the Pharmacy & Therapeutics Society (P & T Society). Dr
Armstrong wrote the chapter entitled "Drugs and Hormones Used in
Endocrinology" in the 4th edition of the textbook Endocrinology. She is an
Adjunct Clinical Instructor of Pharmacy Practice at Midwestern University. Dr
Armstrong currently serves on the Drug Information Advisory Board for
pharmacist.com and on the American Pharmaceutical Association Scientific Review
Panel for Evaluations of Drug Interac ons (EDI).
Morton P. Goldman, PharmD, BCPS, FCCP, He received
his bachelor's degree in pharmacy from the University of Pittsburgh, College of
Pharmacy and his Doctor of Pharmacy degree from the University of Cincinnati,
Division of Graduate Studies and Research. He completed his concurrent 2-year
hospital pharmacy residency at the VA Medical Center in Cincinnati. Dr Goldman
is presently the Director of Pharmacotherapy Services for the Department of
Pharmacy at the Cleveland Clinic Foundation (CCF) after having spent over 4
years at CCF as an Infectious Disease pharmacist and 10 years as Clinical
Manager/Assistant Director. He holds faculty appointments from The University
of Toledo, College of Pharmacy and Case Western Reserve University, College of
Medicine and is the Pharmacology Curriculum Director Coordinator for the new
Cleveland Clinic Lerner College of Medicine. Dr Goldman is a Board-Certified
Pharmacotherapy Specialist (BCPS) with added qualifications in infectious
diseases,
In his
capacity as Director of Pharmacotherapy Services at CCF, Dr Goldman remains
actively involved in patient care and clinical research with the Department of
Infectious Disease, as well as the continuing education of the medical and
pharmacy staff. He is an editor of CCF's Guidelines for Antibiotic Use and
participates in their annual Antimicrobial Review retreat. He is a member of
the Pharmacy and Therapeutics Committee and many of its subcommittees. Dr
Goldman has authored numerous journal articles and lectures locally and
nationally on infectious diseases topics and current drug therapies. He is
currently a reviewer for the Clinical Infectious Diseases and the Journal of
the American Medical Association, an editorial board member of the Journal of
Infectious Disease Pharmacotherapy, and coauthor of the Infectious Diseases
Handbook and the Drug Information Hand- book for the Allied Health Professional
produced by Lexi-Comp, Inc. He also provides technical support to Lexi-Comp's
Clinical Reference Library™ publications.
Dr Goldman
is an active member of the Ohio College of Clinical Pharmacy, the Society of
Infectious Disease Pharmacists, the American College of Clinical Pharmacy (and
is a Fellow of the College), and the American Society of Health-Systems
Pharmacists.
Leonard L. Lance, RPh, BSPharm, has been
directly involved in the pharmaceutical industry since receiving his bachelor's
degree in pharmacy from Ohio Northern University in 1970. Upon graduation from
ONU, Mr Lance spent four years as a navy pharmacist in various military
assignments and was instrumental in the development and operation of the first
whole hospital I.V. admixture program in a military (Portsmouth Naval Hospital)
facility. His last 15 months in the Navy were spent on the USS Independence
CV-62.
After
completing his military service, he entered the retail pharmacy field and has
managed both an independent and a home I.V. franchise pharmacy operation. Since
the late 1970s, Mr Lance has focused much of his interest on using computers to
improve pharmacy service. The independent pharmacy he worked for was one of the
first retail pharmacies in the State of Ohio to computerize (1977).
His love
for computers and pharmacy leads him to Lexi-Comp, Inc. in 1988. He was the
first pharmacist at Lexi-Comp and helped develop Lexi-Comp's first drug data-
base in 1989 and was involved in the editing and publishing of Lexi-Comp's
first Drug Information Handbook in 1990.
As a result
of his strong publishing interest, he presently serves in the capacity of
pharmacy editor and technical advisor as well as pharmacy (information)
database coordinator for Lexi-Comp. Along with authoring the Drug Information
Handbook for the Allied Health Professional and Lippincott Williams &
Wilkins Quick Look Drug Book, he provides technical support to Lexi-Comp's
reference publications. Mr Lance also assists over 300 major hospitals in
producing their own formulary (pharmacy) publications through Lexi-Comp's
custom publishing service. Mr Lance is also Manager of the Dosage Forms
database in the Medical Sciences Division at Lexi-Comp.
Mr Lance is past
president (1984) of the Summit Pharmaceutical Association (SPA). He is a member
of the Ohio Pharmacists Association (OPA), the American Pharmaceutical
Association (APhA), and the American Society of Health-System Pharma- cists
